Catch up!

Sorry for my lack of posts, my millions of fans! I've just been so utterly exhausted with all the call lately. I was on call again on Monday and got very little sleep. Thankfully, I'm not on call again until next Saturday!

I also haven't been to the gym in a while. So, today, after the office, I headed to the gym and did an hour's worth of walking. It felt fine while I was walking, but man oh man, my feet were killing me afterwards. My stomach was also rebelling. So, I laid on my bed in misery until Nasur came home with the kids and taco bell. 

Tonight was also journal club night. I was hosting, so I put out a bunch of cheese and crackers and some wine I picked up. We talked about transgender medicine and how to better serve our transgender patients. After journal club, I cleaned up and am getting ready for bed. I used my plantar fasciitis spiky ball a lot tonight, so I hope my feet recover soon. I need to get some new shoes, but my newish shoes are not super comfortable, so I've been using older shoes that apparently aren't doing the trick. I'm cruising Amazon for other options. We shall see!

Oh, also yesterday was my second visit with my obesity med doc. I've got one more dose of Ozempic at 0.25mg and then will move up to 0.50mg for four weeks. I think it's made a huge difference in my life so far and we discussed that and if I'm having any side effects. We also reviewed my labs and she did say that my fasting insulin was a bit elevated, though it still was documented in the normal range. She said that is contributing a bit to my difficulty in weight loss. Who knew? Makes me feel slightly better that it's not just me failing at losing weight but my basal insulin working against me.

Vacaaaaaation! Also known as GMSTW

Today was an OR day. Thankfully, my foot didn't kill me. It's been doing better every day, so I'm feeling a little stupid that my first response was that I broke it. But, I broke my foot in college doing the exact same thing, so it seemed somewhat of a reasonable assumption. Anywho. I didn't die. Hooray!

Once home, we opted to get some Indian food for dinner and have a family movie night because it's cold and rainy today. Like cold enough that we turned the heat on. Pathetic, Colorado. Pathetic. But, it won't last long. See?


Thankfully, once I was done in the OR, it was the official start of my vacation or my Get My Shit Together Week. I am off until next Monday and I have a long list of things I want to get done. Mostly, it's establishing care with doctors to start taking care of myself better. Monday, I see the optometrist. Tuesday, I'm seeing an obesity medicine doctor. Wednesday, I see the orthodontist. Thursday, I see an internal medicine doc (to establish as my primary care physician). Friday, I get a massage. Friday sounds like the most fun, obviously, lol. I am also going to get my gym membership reestablished and start trying to establish a routine there again, reminding myself that I deserve the time and space everyone else there deserves. 

A few other things on my to-do list:

  • Get a hold of the stupid student loan people
  • Do my PSLF forms
  • Do my maintenance of certification articles
  • Organize journal club for work
  • Put up the wind chime ish thing my mom gave the kiddos
  • Book my mom's flights to come see us
  • Figure out my tax returns
  • Look at my meal prep recipe book and try to find some new recipes to try
  • Figure out how to meal prep
  • Figure out outfits for family photos next weekend

I am glad to have some time off and not being going anywhere or doing anything major that I can't be flexible about. I'm also nervous af to be making some bigger commitments to myself. Here goes nothing!

Bit of a pity [food] party.

Today started out okay, but as I felt my foot throbbing, I slowly spiraled into a pity party. It went something like this: If I can't move more, how do I lose weight? If I can't lose weight, how am I ever going to be healthy? If I can't be healthy, how am I ever going to be happy? Well, I'm never going to lose weight. So I'll never be happy. So, may as well eat my feelings.

Cue: overeating. I had a healthyish breakfast of pesto toast (I made my first ever pesto this weekend) with some tomatoes and then as lunch got closer and closer, I was like I'll order out for lunch. I got a gyro + fries + pita chips + feisty feta dip. I ate all of that food slowly from 12:30 - 5:30, but I ate all of it and definitely didn't need to eat all of it.

So, I ended up feeling worse than my foot did. 

Once home, I skipped out on dinner because I wasn't hungry and then took the kiddos for a walk. Nasur wanted to stay home and clean. Reyah rode her bike and I pulled Zayd in the wagon. No one else was out because the skies looked ominous. But, we lucked out and did our whole walk without any rain. We did bath time and bed time and now I'm trying to think of how to keep myself motivated despite this setback. I felt fine last night, but I'm frustrated today. I'm also worried about tomorrow because it's an OR day and I'm not sure how standing all day on my foot is going to feel. Le sigh.

A little set back

 So, yesterday I was on call and I was hurrying to get to the OR to do a C section and I heard my foot crack. As I age, more and more thinks are cracking and snapping, so I thought nothing of it. Once back in my call room, I wanted to try to get in as many steps as possible before midnight. I knew I likely wouldn't get to 10,000, but I wanted to get as close as possible. After getting about 2500 steps, I had to stop because my left foot was hurting. I didn't think anything of it and went to bed. I woke up in the morning and it was painful to bear weight on it. I was like hmm. That's not great. I made an appointment at urgent care for later in the afternoon and went home.

Things didn't improve at home, so I went off to my appointment and got an x-ray. Nothing broken, so that's good news, but I'm still having pain with weight bearing. The nurse practitioner said maybe I had a sprain. But, I've got no bruising or swelling, so I doubt that's likely either. Nevertheless, the pain is real. So, I've decided to give myself the time I need. I will keep off it so it'll heal faster from whatever it is that I did to it and I will redouble my efforts with my eating so as to offset my less than ideal movement. 

It's probably good timing because I was on call on Saturday and yesterday, and had dinner and drinks out on Friday and had folks over for one last summer shindig on Monday, so my eats have been less than stellar. I have, however, been good about watching my portions, and I've not overdone it on the sweets. But, still, not stellar. 

So, onwards to better eats and calorie tracking and being nice to my foot!

Humbled

Today was an office day. We were at a different office than yesterday, which was much less conducive to teaching and comfort (i.e., no chairs for me to sit in during patient visits). My hips were hurting a bit at the beginning of the day and my foot continues to act up. I have a hard time believing that it's some type of foot sprain given how long the pain has been going on. However, I can't recall any type of injury, so I don't see why I'm having pain in the first place. I'm thinking that if I'm given the opportunity/time, I might head to an urgent care tomorrow night after work. That way I'll have an x-ray that can definitively say either yes, it's a fracture or no, it's all muscular. 

Anywho, on to the next subject. The doctor I'm currently working with specializes in urologic oncology: kidney cancer, prostate cancer, bladder cancer, etc. A lot of our day is spent meeting with patients and following up PSA levels, which is a marker of prostate cancer. Today we met with a patient who was newly diagnosed with prostate cancer and I was extremely honored to be able to be there for that conversation. He was searching for answers and recommendations on what to do. Given that prostate cancer doesn't follow a predictable pattern, the patient could go 20-25 years without any problems, or he could start having urinary and prostate problems within 5 years. It must be an absolutely terrifying to get such a diagnosis and the fact that he was so kind to me (a student who's just trying to learn by observation and listening) meant a lot. The doctor also shared his own experience with prostate cancer to help put the patient at ease and you could see the patient visibly relax knowing that his physician truly understood what he was going through.

I have an OR day tomorrow, which I'm not so excited about. And since I know I'm not going to be in any shape to hit the gym afterwards, I'm going to hit the gym beforehand. Thankfully, that only means a 6 a.m. wake-up rather than a 7:30 a.m. one. I can do that. So, I'll do it. Tomorrow - bright and early gym time. It's been a while since I've done any strength training so it'll probably be a bit rough, but I'll be fine :-)
